2017 Van Delivery Survey December 4-8

Hello, everyone.  The state library’s van delivery service needs your help!  Because the van service is financed with LSTA funding, our annual report to the government must explain how these funds are used.  So we’re asking that every library participate in this year’s Van Delivery Statistics Survey, the week of December 4-8, 2017.

The survey just requires a simple count of items that you send out on each of your van pickup days.  

The online survey is available online at:    http://tinyurl.com/nhvans2017.             

Please print the form, fill it out on your van pickup days the week of December 4 - 8, and return it by van OR by email to Charles.Shipman@dncr.nh.gov.  

If you deliver your items to another library for pick up, please count these items before dropping them off and complete the statistics form using those numbers.
