N.H. Department of Revenue Administration - State Tax Forms

The New Hampshire State Library is pleased to provide to libraries New Hampshire tax forms from the NH Department of Revenue Administration (DRA).

The DRA is providing one copy of Package X and one copy of the Interest and Dividends tax form and instructions (DP-10) to each library who received them in past years. Unfortunately, they can no longer provide multiple copies of their tax forms for distribution.

For those libraries who have received them in the past, when you get your packet, please use the forms as a master copy which your patrons can consult. You can also obtain copies from the NH Department of Revenue website at

Your patrons can contact the Department of Revenue directly for information or other tax forms.

State Tax Forms - DP-10 and DP-10 Instructions

The second shipment of N.H. Department of Revenue Administration tax forms are being sent out on the van this week.

This second shipment contains the Interest and Dividends Tax Form (DP-10) and Instructions (DP-10-Inst).

Libraries requesting a large number of forms may find their numbers have been reduced from last year. If a patron needs additional forms you may photocopy from Package X, print the forms from the Department of Revenue Administration's web site, http://www.revenue.nh.gov/forms/index.htm, or direct the patron to call the Forms Line maintained by the Department of Revenue Administration (603)271-2192.

Package X is being sent out via van delivery. When you receive your packet, please take a moment to look at the address label and let me (diana.decota@dcr.nh.gov) know of any corrections (contact person, etc.) that need to be made.

N.H. Department of Revenue Administration Tax Forms

A shipment of NH Department of Revenue Administration tax forms is being sent on the van to those libraries who have requested them in the past.

This shipment contains the Interest and Dividends Tax Form (DP10) and Instructions (DP-10-Inst) and a Quick Checklist (DP-10-Checklist).

Libraries are receiving the number of copies they have requested in the past of the Interest and Dividends Tax Form (DP-10). We did not receive an adequate number of the Instructions (DP-10-Inst) and the Quick Checklist (DP-10-Checklist) to distribute the number requested in previous years. In all cases, at least one copy has been included. If more of these become available, we will provide you with more copies.

We are also waiting to see if we receive an adequate number of copies of Package X which contains all the forms and instructions issued by the Department of Revenue Administration. This will be sent in a separate mailing if these are made available to us.
